The SCO GSSC will provide initial assessment and ongoing assessments of the SCO client's needs including eligibility for any relevant **MEMBERS ONLY**SIGN UP NOW***. (BSHC) service and other community resources. Furthermore, the SCO GSSC will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment. This position requires the ability to work independently, multitask, conduct home visits and family/provider meetings, telephone contact and complete regular documentation. The SCO GSSC is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, SCO representatives, vendors, potential referral sources and other community agencies. SCO GSSCs actively participate in BSHC organizational life.nu00a0nEssential Functions:nEssential Job functions: Duties _______ through _______are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions. TBDnThrough home visits conduct multi-dimensional assessment of elders' functional and social needs incorporating eligibility/need for services, special programs and other community resources. nProvide ongoing assessment of client needs/status through contact with collaterals, informal supports and contact with the client via phone and regularly scheduled home visits. Adjust service planning and follow-up as indicated. nThrough a needs assessment, knowledge of resources, and problem solving skills develop a service plan that incorporates the areas identified within the assessment. Seek supervision for ongoing case management direction, high-risk client issues, vendor needs/issues, etc. nInitiate and follow up with an interdisciplinary consultation and review with SCO personnel for SCO GSSC's service planning recommendations and obtain approval of those services. u00a0 u00a0 u00a0 Implement the plan utilizing all available resources; BSHC vendors/programs and other community resources.nMonitor the efficacy and quality of services provided by BSHC through contacts with client, their family, collateral contacts and vendor monitoring. nMaintain the client record through timely documentation that is complete and accurate of all related communications and required forms. nProvide ongoing advocacy for client, as follows:nExhibit working knowledge of BSHC services/programsnExhibit working knowledge of other community programsnContinually assist in the development of community resources knowledge within BSHCnSeek peer or supervisory consultation to explore unfamiliar resource areas. nnContribute to organizational life through active participation in team activities, committees, and work groups. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.nPhysical Demands:nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.nThe employee must be able to carry a 15 to 20 lb. loadnThe employee is regularly required to talk or hearnThe employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. nPosition Type/Expected Hours of Work:nThis is a full time position.
