Health Homes Manager
Job Overview
The Health Homes Program (HHP) is designed to serve eligible Medi-Cal beneficiaries with multiple chronic conditions who are frequent utilizers and may benefit from enhanced care management and coordination. The HHP coordinates the full range of physical health, behavioral health, and community-based long-term services and supports (LTSS) needed by eligible beneficiaries.
The **MEMBERS ONLY**SIGN UP NOW***. Health Homes Manager (HHM) has oversight of the entirety of the Health Homes Program and care coordinators. The HHM ensures a seamless continuum of care for HR360 patients. The HHM also ensures collaborating with the HR360 multidisciplinary care team as well as external referral sources to coordinate the patient's care.
Key Responsibilities
Leadership Responsibilities:
Able to read multidisciplinary team in providing seamless continuum of care and coordination for high need, high risk populations.
Champions quality improvement efforts and works with the multi-disciplinary and QI team to achieve quality outcomes.
Acts a liaison with other HR360 departments and programs to create and maintain systems of care coordination.
Acts a liaison with external referral resources and programs to create and maintain systems of care coordination.
Management responsibilities:
Oversees team of care coordinators at various sites.
Sets up and ensures systems of care coordination to enhance outcomes for high-risk patients.
Care coordination responsibilities:
Helps to create and provide oversight for care coordination activities.
Works with care coordination and healthcare teams to ensure a Health Action Plan (HAP) is created for all Health Homes patients and other patients requiring care.
Ensures that goals of individual patients HAP are being met.
Education and Knowledge, Skills and Abilities
Master's degree from accredited college or university or equivalent experience
Experience managing multi-disciplinary care teams
Understanding of high need patient population
Experience working homeless, criminal justice, or other underserved populations
Experience with trauma informed care and harm reduction
Understanding of social determinants of health and impact on patient well being
Must not be on active parole or probation
Culturally competent and able to work with a diverse population
Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data
Professionalism, punctuality, flexibility, reliability are imperative
Excellent verbal, written, and interpersonal skills
Integrity to handle sensitive information in a confidential manner
Action Oriented
Strong problem-solving skills
Excellent organization skills and ability to multitask and juggle multiple priorities
Outstanding ability to follow-through with tasks
Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility
Strong initiative and enthusiasm and willingness to pitch in whenever needed
Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations
Able to work within a frequently changing project scope while maintaining overall direction and structured priorities
Desired:
Social work degree or nursing degree
Bilingual in Spanish
Knowledge of co-occurring disorders and trauma informed treatment
Knowledge of clinical documentation (treatment plans, progress notes etc.)
Experience working with criminal justice population
