Monterey Park Hospital is recruiting for a Dischare Planner in our Utililzation Review Department. This is a full time, 8 hour, day shifts. This position currently reports to the Chief Operating Officer.nResponsibilitiesnThe Discharge Planner is responsible for delivering social services to patients including psychosocial evaluation and advocacy, teaching, submitting referrals to community services and provides assistance, as needed, with discharge planning. The Discharge Planner interviews patients, makes social assessments, and interacts with families, significant other, physicians and staff, to obtain information that will assist with the patient's ability to understand his or her medical condition(s). The Discharge Planner contacts insurance companies and vendors to make discharge planning arrangements. This position makes recommendations for additional services for the patient upon discharge and ensures documentation in the medical record is clear and complete.nQualificationsnHigh school diploma required. 1 year working in an acute care facility required. 2 yearsu2019 experience working in an acute care facility or similar health care position preferred. Computer proficiency with Microsoft Office. Knowledge of CMS, Medicare/Medi-Cal required. Knowledge and experience with The Joint Commission, CMS and CDPH requirements. Excellent written and verbal communication skills in English. Bilingual English/Spanish skills to communicate effectively with patients and families preferred. Ability to establish and maintain effective working relationships across the organization.
